Please enter in Latin letters, exactly as in the international passport of the person who will sign the contract.
If the representative does not have a PESEL number, please provide the series and number of the foreign passport, as well as the date of birth.
If you do not live in Poland, please write your residential address in your home country using Cyrillic: street, house/apartment number, postal code, city, country.
Has the NIP-8 form already been submitted to the tax office?
If the account has not been opened yet, please leave this field empty.
If no foreign currency accounts exist, please leave the field empty. If there are several, please indicate which is EUR, USD, etc.

Fee: 400 PLN
Do you need to prepare a PCC-3 declaration (tax on share capital)?
Your first tax after company registration amounts to 0.5% of the share capital value.
